User locked after three unsuccessful login attempts

You will be locked out of the system after three incorrect login attempts. Once

your account has been locked, you may reset your password to unlock the account.

If you are unable to remember your username:

o Click the link on the login page.

o Enter the e-mail address associated with your Dynamic Forms account.

o Provide your last name.

Dynamic Forms will return your secret question.

o Simply answer the secret question correctly & your username will be sent to the e-mail address associated with your account.
