Application Form for Admission to SIL-UND (Stage 2)

This form is part of the second stage of applying to SIL-UND. It asks for information that helps us prepare for your arrival.

It assumes that you have submitted the stage 1 application form and have been accepted as a student at SIL-UND for the 2008 summer session. (If not, please read the instructions at www.und.sil.org/apply.htm and then proceed to the stage 1 form.)

You can submit this stage 2 form electronically or by mail/fax:

  • Electronically (preferred): You must complete the entire form in one session, which takes 15-30 minutes; there is unfortunately no way to save your work between sessions. The form works best if you have scripting enabled in your browser.
  • By mail or fax: Complete the form on your computer and print it out; or print it out first and fill it in by hand. Then send it to us as explained at the end of the form.

Deadlines for submitting this form and your SIL application fee:

  • International students should submit them in time to arrive to SIL by March 1. (More time is required to obtain immigration documents from some countries, so the sooner you submit these things, the better.)
  • Students who indicated on their stage 1 application that they would like to apply for a scholarship must send them so they arrive by April 1 to preserve eligibility for financial aid.
  • All other students, please send them so they arrive by May 1; submissions after that date will result in a late fee (added to your bill when you register for classes in June).

Your directory information

To facilitate communication among staff and students, SIL-UND distributes a directory each summer with rooms, phone numbers and email addresses of adults (not children). Directory information is primarily for use by SIL participants and is not published for use outside the program. Occasionally, though, we receive requests for this information by people outside the SIL program (e.g., a relative, a UND staff member, or a non-profit organization with whom the participant has a relationship), and we use the directory as a basis for answering such requests. Most people want to have their directory information included in this list, but you may choose to have some or all of your information excluded.

We eventually need Social Security numbers for all students in order to interact with the University and to issue tax-related forms (1098-T) at the end of the year. We use SS#s only for administering the SIL-UND program and we guard them to keep them confidential.

Please note: This form is only minimally secure. SS#s are disguised when information is transmitted to us but they are not encrypted. If you have any hesitation about submitting your Social Security number(s) electronically, do not do so; instead, mail it/them to us with your check or call us at 800-292-1621 and leave a message on our answering machine (see also contact information).

Your choices about housing and meals

Most participants choose to live in the dormitory complex with other SIL staff and students and eat meals in the university cafeteria. (For more information, see www.und.sil.org/roombrd.htm.)

The cost for housing is kept low, in part, by students who live in the dorm contributing an hour of work each day to the program. If you do not want to participate in this arrangement, you may either pay an additional charge (currently $390, see costs) or make outside arrangements for housing and meals.

Your children (if any)

All children 13 years of age and under who are living in the dorm must be supervised at all times. When parents are in classes, this is normally done through our child care program. The housing and meals plan is intended for SIL students and staff and their minor dependents. Permission from the director of the program is necessary for a spouse or adult child to live in the dormitory and eat in the dining room if he/she is not participating actively in the SIL program as a student or staff member. If you need to request such permission, you may do so here (or see our contact information).


   


Special accommodations and assistance

Some people require special accommodations or assistance in order to participate in the SIL-UND program. Examples:

  • first-floor housing
  • sign language interpreters
  • note-takers
  • readers

We will make reasonable efforts to provide them, in accordance with UND policy. Although you may make requests at any time, it will help us help you if you make your request now rather than later. All requests are kept confidential. (If you prefer not to submit this information electronically, please send it by mail with your check.)

After you submit this stage 2 form, please send us a check or money order, payable in U.S. dollars to "Summer Institute of Linguistics", for the following amounts:

  • $25 application fee per student (nonrefundable)
  • $100 housing deposit per adult, if you want to live in the dormitory and are not on staff (refundable before May 1, and applied toward your housing charges once you arrive)

One check covering both amounts is fine. (Spouses may combine all amounts on one check.)

Please also include your U.S. Social Security number, if you have one.
