This year, University Relations will get a download of employee information from the Payroll Office to complete their directory, instead of requesting employees to complete cards with their information. The work address, work phone number(s), work location and work email address - as it is listed in the online directory, will be included for all benefitted employees. Temporary and student employees will not be included in the employee upload. Home address and home telephone number will be also be listed, unless the employee has requested in writing that home information not be included.
Employees will be asked to verify that their campus (work) and home address is correct as listed on the memo. If not, they will be requested to enter the correct information and return the memo to the Payroll Office. If the employee does not want their home information in the printed directory, they are to initial the request, sign, date and return the form to the Payroll Office. Even if the employee chooses not to have their Home address listed in the directory, they should include any corrections, so W-2's can be sent to the correct
address.
If your information is correct and you want your home address and telephone number in the printed directory - PLEASE DO NOT RETURN THE FORM. |
